Declaration of consent for processing of personal data

Pursuant to the Regulation of the European Parliament and of the Council (EU) 2016/679 of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and the repeal of Directive 95/46 / EC (Official Journal of the EU No. 119, page 1) – art. 12, art. 13. informs that:
I hereby express my consent to the processing of my personal data included in form by Foundation Zumbi Cycles with its registered office Bielsko Biała (32- 400), Poland, for the purposes, execution of realization form, marketing and programme regarding goods offered by the Foundation and its associated entities, including marketing programme and execution of form requirements of the Foundation. I also give my consent to execution of form requirements.
The consent for the processing expressed above includes the following personal data: name and surname, e-mail address, phone number, personal preferences and more information.
I hereby represent that I have been informed about my right to:
1. access and adjust my personal data,
2. lodge written, motivated request to cease processing my personal data,
3. lodge objection against processing my personal data,
4. as well as that providing my personal data is voluntary, but necessary to realization selling process

Declaration of consent for receiving commercial information in electronic way:
Pursuant to Article 10 paragraph 2 of the Act on Rendering Electronic Services dated 18 July 2002 (Journal of Laws of 2002, No. 144, item 1204 as amended) I hereby express my consent to receive from Foundation Zumbi Cycles with its registered office in Bielsko Biała, Poland programme information in electronic way.

Information about cookie files

1. The Website uses cookies.
2. Cookie files (so-called “cookies”) are computer data, mainly text files, that is stored in end-devices of the Website User and is intended for use with the pages of the Web Service. Typically, cookies contain the name of a website from which they come, their storage time on end-devices and a unique number.
3. The Website Operator places cookies in Website Users’ end-devices and has direct access to them.

Server logs.
1. Information about some user behaviour is subject to logging-in into the server layer. This data is used only to administer the Website and provide the most efficient use of hosting services.

3. The aforementioned data is not associated with individual persons viewing the pages.

Providing data
1. The data can be made available to third parties only within legally allowed limits
2. The data that enables identifying a given user is made available only upon the user’s consent.
3. The Operator may be required to provide the information gathered by the Website to authorised bodies based on legitimate demands arising from the demand.
